About Us:
APT Foundation is a private, nonprofit organization dedicated to improving the health and well-being of individuals and families affected by substance use and mental health disorders. Serving communities across Connecticut since 1970, we provide comprehensive, evidence-based treatment, including medication-assisted treatment, counseling, mental health services, primary care, and recovery support.

Job Summary:
Under the direct supervision of the VP, Clinical Operations process information requests for medical records in a timely and efficient manner. These requests include disability applications, attorney generated requests for information to aid in processing patient disability requests and/or legal matters; outside provider requests for information; and patient requests for copies of their records.
Essential Duties and Responsibilities:
Complete requests for information within 48 hours of receipt. Document disclosures in the APT Foundation EHR. Return phone calls for requests within 24 hours of receipt Assists in document scanning of external documents to the APT Foundation EHR. Assist in data entry into the DMHAS Ddap system. Provides ongoing coverage to the Call Center. Provides project assistance to various internal projects, both short and long term, as time permits.
Qualifications and Skills:
Bachelor's Degree or equivalent work experience Desire to work in a fast paced, changing environment Desire to work with persons who have substance use and mental health disorders Knowledge of excel and other data-based systems Understanding of basic medical office functionalities Considering the unique relationship within multiple departments, it is estimated that in the typical day, the approximate time allocation is: 3.0 hours/day Medical Records Coverage 3.0 hours/day Call Center 1.5 hours/day
