Job Description

Job Overview We are seeking a dedicated and compassionate RN Admissions/Intake Nurse to join our dynamic healthcare team. In this vital role, you will be the first point of contact for patients entering our facility, conducting comprehensive assessments and coordinating their admission process. Your expertise in critical care, hospital medicine, and diverse clinical settings will ensure smooth transitions from outpatient clinics to inpatient units, providing exceptional patient-centered care from the moment they arrive. Duties Perform thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine appropriate care pathways. Coordinate admission and intake procedures across inpatient, outpatient, emergency room, and post-acute care settings, ensuring compliance with hospital policies and HIPAA regulations. Collaborate with multidisciplinary teams to facilitate discharge planning, diagnostic evaluations, and case management strategies tailored to each patient's needs. Review medical records and documentation for accuracy, completeness, and adherence to medical terminology standards. Conduct utilization management by evaluating patient eligibility for services and managing resource allocation efficiently. Assist in diagnostic evaluations by gathering relevant clinical data and supporting physicians during initial patient encounters. Maintain detailed records of patient intake processes, ensuring confidentiality and security of sensitive information. Experience helpful but not necessary Proven experience in critical care environments such as ICU or Level II Trauma Centers with exposure to emergency medicine or Level I Trauma Centers. Demonstrated expertise in hospital medicine, medical-surgical units, or outpatient clinics with a focus on acute care and inpatient services. Prior involvement in infectious disease care, behavioral health settings, pediatrics (including PICU), geriatrics, or post-acute care is highly desirable. Strong knowledge of physiology, medical terminology, patient assessment techniques, and discharge planning processes. Experience working within managed care systems or utilization management programs is a plus. Familiarity with electronic medical records systems and medical records documentation standards. Excellent communication skills with the ability to perform comprehensive patient evaluations in fast-paced environments such as emergency rooms or outpatient clinics. Pay:
$83,000.00 - $93,000.00 per year
