This vital role involves developing comprehensive, compliant, and patient-centered plans of care for home health services in accordance with Medicare regulations and clinical standards. The ideal candidate will possess extensive experience in medical documentation, utilization review, and clinical care coordination, ensuring that each patient's plan of care promotes optimal health outcomes and adheres to regulatory guidelines. Duties QA and review OASIS and develop individualized Home Health 485 Plans of Care that accurately reflect patient assessments, clinical needs, and treatment goals, ensuring compliance with Medicare requirements. Review medical records, clinical notes, and documentation from various sources to verify accuracy and completeness prior to plan submission. Utilize Igea Software to document, update, and maintain plans of care efficiently within the healthcare information system. Ensure all plans incorporate appropriate coding such as ICD-10 codes for accurate billing and reimbursement. Qualifications Proven experience in home health a must. Strong knowledge of Igea Software.
