Essential Responsibilities:
1. Chairs the daily Treatment Team Planning Meeting. 2. Completes the treatment team progress note and places it in the appropriate patient's chart. 3. Facilitates and expedites discharge planning with attending staff and utilization management to ensure continuity of care for outpatient follow-up. 4. Educates the patient and/or family on disease, medication side-effects and healthy lifestyle options. 5. Helps ensure the coordination and care of patients in close collaboration with the physician and other healthcare personnel. 6. Communicates with the attending physician and other health care team members on a regular basis regarding the patient's discharge plan. 7. .Provides statistical information and other reports as requested. 8. Attends hospital and medical staff meetings as designated by the Director of Social Services 9. Attends in-service and professional education programs. 10. Serves as consultant and mentor to staff in solving problems. 11. Delivers clear, convincing, and well-organized presentations. 12. Other duties as assigned.
