DISCHARGE PLANNER

Responsibilities The Discharge Planner is responsible for coordination of discharge planning services within the Social Services Department. These duties include but are not limited to daily audit to ensure effectiveness of clinical plans, verification of communication with appropriate supports and partners, direct discharge planning and case coordination assignments, provision of reports and development of discharge process improvement as directed by the Director of Clinical Services. Qualifications:
