Job Description
Overview Join our dynamic healthcare team as a
CAC/CSC/ADT
professional, where your expertise will directly impact patient care and hospital operations. In this vital role, you will serve as the first point of contact for patient admissions, discharges, and transfers, ensuring seamless communication between clinical teams, patients, and external providers. Your energetic approach and attention to detail will help facilitate efficient patient flow within a fast-paced hospital environment. This position offers an exciting opportunity to utilize your skills in acute care settings, including Level I Trauma Centers, Level II Trauma Centers, and specialized units such as PICU and outpatient clinics. Your dedication will support optimal patient outcomes and enhance overall hospital service quality. Responsibilities Manage patient intake processes by collecting comprehensive medical histories, verifying insurance information, and ensuring accurate documentation in medical records. Coordinate discharge planning by collaborating with multidisciplinary teams to arrange post-acute care services, outpatient appointments, and community resources. Conduct thorough patient assessments to determine care needs, utilizing knowledge of physiology, medical terminology, and behavioral health considerations. Facilitate utilization management activities by reviewing diagnostic evaluations, case management documentation, and ensuring compliance with HIPAA regulations. Assist with inpatient and outpatient case management tasks, including post-acute care coordination and outpatient clinic scheduling. Support social work functions by providing mental health counseling referrals and addiction counseling when appropriate. Maintain detailed records of all patient interactions, discharge summaries, and utilization review activities to ensure regulatory compliance. Skills Extensive hospital experience with a strong background in acute care environments such as ICU, emergency medicine, or trauma centers. Knowledge of Level I and Level II trauma center protocols, pediatric (PICU) care, and post-acute care processes. Familiarity with medical terminology, physiology concepts, diagnostic evaluation procedures, and inpatient/outpatient workflows. Proficiency in patient assessment techniques combined with excellent communication skills for effective intake and discharge planning. Experience with medical records management, utilization management systems, and case management software. Understanding of HIPAA regulations to maintain patient confidentiality at all times. Ability to work collaboratively within multidisciplinary teams including social workers, behavioral health specialists, nurses, physicians, and managed care providers.
