Job Description

Lincoln Surgical Hospital is an innovative healthcare leader in providing exceptional surgical services for our patients. Our dedicated physicians and staff strive to achieve optimal surgical outcomes in a patient focused atmosphere. We are looking for a Health Advocate/Case Manager to join our team.
Hours:
PRN (as needed), 8 am - 4:30 pm. Job duties include but are not limited to: Assist patient in the development of a plan of care for ongoing and maintenance improvement of health care. Work to resolve clinical, psychosocial and financial barriers on the patient's behalf. Responsible for performing utilization review and discharge planning in accordance with all mandated regulations. Perform early intervention during patient stay regarding the disposition of patient. Participate in multidisciplinary patient rounds as indicated. Assure appropriate allocation of facility resources through monitoring and reporting on utilization of services. Liaison with Call Center staff in identification of and proactive solutions for pre-operative problems. Assist with pre-operative patient education for the total joint replacement classes. Assist on post-surgical units with non-clinical and clinical tasks based on training, competency and licensure as needed. Review Inpatient Services daily admissions. Assist physicians as needed with information regarding patients and appropriateness of inpatient versus outpatient/observation status. Concurrent and retrospective records review, including patient admit and discharge status and discharge plan of care. Assist in providing quarterly utilization review reports. Current State of Nebraska RN licensure required. Current BLS required at time of hire. Prefer experience in utilization review, case management and/or discharge planning.
