Job Description

Job Overview We are seeking an experienced and detail-oriented
RNAC/MDS
Coordinator to join our skilled nursing team. The RNAC is responsible for coordinating and completing resident assessments, ensuring regulatory compliance, supporting care planning, and maximizing reimbursement through accurate MDS documentation. The role works closely with nursing, therapy, social services, and interdisciplinary team members to promote quality resident care.
Responsibilities:
Coordinate and complete MDS assessments and RAI processes accurately and on time. Develop and maintain individualized resident care plans. Ensure compliance with Medicare, Medicaid, OBRA, and state regulations. Participate in care plan meetings, utilization reviews, and interdisciplinary team meetings. Review documentation to support accurate coding and reimbursement. Monitor residents for changes in condition and communicate findings to the care team. Assist with Medicare and managed care case management as needed. Support quality assurance and regulatory survey readiness.
Qualifications:
Current Pennsylvania RN license required. Previous
RNAC/MDS
experience preferred. Knowledge of PDPM, Medicare, Medicaid, and long-term care regulations. Experience with PointClickCare (PCC) preferred. Strong organizational, communication, and analytical skills. Ability to work independently and collaboratively within an interdisciplinary team.
