Job Description

Overview Join our compassionate team as a Hospice Medical Social Worker (MSW), where you will play a vital role in providing holistic, patient-centered care to individuals facing life-limiting illnesses. In this dynamic position, you will assess emotional, social, and psychological needs, develop tailored care plans, and support patients and their families through challenging times. Your expertise will help ensure comfort, dignity, and quality of life for those in our hospice program. Duties Conduct comprehensive patient assessments to evaluate emotional, social, and behavioral health needs within hospice care settings. Develop and implement individualized care plans that incorporate clinical counseling, behavioral therapy, and family support strategies. Provide individual and group counseling sessions focused on grief, loss, addiction counseling, or mental health challenges such as PTSD or dementia-related issues. Collaborate with interdisciplinary teams to coordinate services including crisis intervention, case management, and post-acute care planning. Maintain accurate medical documentation in compliance with HIPAA regulations and ensure timely updates of medical records using Electronic Medical Record (EMR) systems. Assist with intake procedures for new patients and facilitate communication with healthcare providers regarding diagnosis codes (ICD-10/ICD-9), CPT coding, and other medical terminology. Offer support for families dealing with Alzheimer's care, working with individuals with developmental disabilities or autism spectrum disorders as needed. Qualifications Master's degree in Social Work (MSW) from an accredited program; licensure or certification required per state regulations. Proven experience in hospice care or home & community care environments; inpatient or nursing home experience is highly desirable. Strong knowledge of medical documentation standards, HIPAA compliance, ICD coding (ICD-10/ICD-9), and EMR systems. Familiarity with clinical research methodologies and data collection techniques relevant to behavioral health and palliative care settings. Expertise in child & family counseling, crisis management, cognitive behavioral therapy (CBT), motivational interviewing, and psychotherapy modalities. Ability to work effectively with diverse populations including seniors, children, individuals with disabilities or developmental disabilities, and those requiring Alzheimer's or PTSD care. Excellent communication skills for intake assessments, diagnostic evaluations, group therapy facilitation, and interdisciplinary collaboration. $40.00 - $41.00 per hour
