Qualifications Driver's License Full Job Description Summary of Position:
The purpose of this position is to provide leadership to the clinical team in planning a patient's discharge. The case manager is the member of the clinical team who is responsible for identifying the needs of the patient as they prepare for discharge, and is responsible for identifying resources in the community that may be used to meet those needs. Case managers assist the patient in breaking the cycle of homelessness by connecting them to any available housing options for which they may qualify. This is a safety sensitive position.
Essential Duties:
Works as part of the clinical team with the primary responsibility of discharge planning Attends daily team huddles when in clinic, clinic staff meetings, and team meetings Maintains appropriate documentation in the medical record so that the case management plan for each patient is easily accessible Works with each patient on a comprehensive plan for discharge to permanent housing options Fosters empowerment and recovery by connecting the patient with community agencies to obtain services such as vocational rehabilitation, mental health and substance abuse services, job readiness and parenting skills Accompanies patients on site visits with the goal of clarifying the discharge plan Counsels patients on entitlements for which they may qualify and facilitates applications Keeps the clinical team updated on progress of each patient towards discharge Collects data accurately for all grants and projects, as directed by supervisor
Supervisory Responsibility:
None Qualifications:
Associates Degree or equivalent experience required; BSW preferred 3 - 5 years of experience preferred Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors Experience in working with culturally diverse, chronically homeless, and medically needy individuals preferred. Demonstrates excellent oral and written communication skills Bilingual skills - Spanish highly desirable Must demonstrate critical thinking, problem-solving, and organizational and time management skills Possess and maintain a valid Arizona driver's license and has use of privately owned vehicle Physical and Mental Requirements Position requires extended periods of sitting and standing including bending and reaching Handles stress in a positive manner Ability to be flexible and to multitask Demonstrated interest in working with an underserved population
