Our residential programs offer a supportive, restorative environment focused on evidence-based therapies, life skills development, and long-term recovery success. Position Summary We are seeking a dedicated Residential Treatment Case Manager to join our multidisciplinary team. In this vital role, you will oversee all aspects of case management for clients in our residential treatment programs. You will serve as a primary advocate and coordinator, helping clients navigate their recovery journey by addressing clinical, practical, and transitional needs. Key Responsibilities Conduct comprehensive intake assessments, including interviewing clients and families to identify needs, strengths, and barriers to recovery. Develop and implement individualized service plans in collaboration with clinical, medical, and residential teams. Coordinate care across residential treatment, sober living, outpatient services, and community resources (housing, employment, education, legal support, etc.). Facilitate discharge planning and aftercare to promote successful community reintegration. Maintain accurate, timely documentation in compliance with regulatory standards (e.g., Joint Commission, state licensing). Advocate for clients with insurance providers, courts, social services, and other stakeholders. Participate in team meetings, treatment reviews, and crisis intervention as needed. Build strong relationships with clients to support engagement, motivation, and progress toward recovery goals.
Qualifications Required :
Fingerprint Clearance Card, Negative TB test, CPR certified. At least 1 year of experience in case management, preferably in residential treatment, substance use, or mental health settings. Strong knowledge of co-occurring disorders and recovery-oriented systems of care. Excellent communication, organizational, and problem-solving skills.
