In this vital role, you will coordinate and oversee the care of injured workers, ensuring seamless communication between healthcare providers, employers, and insurance carriers. Your expertise in acute care settings, including critical care, trauma centers, and hospital medicine, will drive effective case management and promote optimal recovery outcomes. This position offers an exciting opportunity to make a meaningful difference in patients' lives while utilizing your comprehensive medical knowledge and exceptional patient service skills. Header Duties Conduct thorough patient assessments to evaluate injury severity, medical needs, and appropriate treatment plans. Coordinate inpatient and outpatient care, including discharge planning and post-acute services, to facilitate smooth transitions across care settings. Collaborate with multidisciplinary teams in Level I Trauma Centers, Level II trauma centers, PICUs (Pediatric Intensive Care Units), and emergency departments to develop individualized care strategies. Manage utilization review processes by evaluating diagnostic evaluations, medical records, and treatment protocols to ensure appropriate resource use. Facilitate communication with healthcare providers across specialties such as infectious disease care, geriatrics, behavioral health, hospital medicine, and surgical units like OR (Operating Room) experiences. Oversee patient documentation while maintaining compliance with HIPAA regulations and ensuring accurate medical records management. Engage in intake procedures for new cases, perform vital signs assessments, and conduct ongoing patient care monitoring using physiology knowledge to support recovery efforts. Header Qualifications Proven experience in critical care environments such as ICU (Intensive Care Unit), emergency room settings, or trauma centers. Demonstrated expertise in hospital medicine, medical-surgical units, or post-acute care facilities. Strong background in managed care principles with a focus on utilization management and discharge planning. Familiarity with diagnostic evaluation processes and medical terminology essential for effective case management. Knowledge of patient assessment techniques including vital signs monitoring and physiology fundamentals. Experience working with diverse populations including pediatrics (PICU), geriatrics, or outpatient clinics is highly desirable. Excellent communication skills for coordinating multidisciplinary teams and engaging with patients empathetically.
