We are seeking an In-Office Clinical Case Manager who operates with urgency, precision, and strong clinical judgment. This role is central to ensuring clean, compliant, and timely patient care from admission through discharge. You will act as the clinical control point between field staff, physicians, and internal departments — driving quality, reducing delays, and maintaining regulatory excellence. This is not a passive role. We are looking for someone who takes ownership, solves problems quickly, and keeps cases moving.
Requirements:
Active, unrestricted California RN License Minimum 3 years of Home Health experience (required) Strong OASIS knowledge with proven accuracy Experience in case management, clinical review, or QA (preferred) Proficiency in EMR systems (WellSky/Kinnser required) Strong understanding of Medicare guidelines and Conditions of Participation Excellent organizational skills with ability to manage high case volume Strong communication skills with physicians and clinicians High accountability, responsiveness, and attention to detail Performs other duties and activities as delegated by the Clinical Director. Multi-lingual candidate - English, Spanish, Armenian Responsibilities:
Own and manage patient cases end-to-end — admission through discharge Review OASIS, plans of care, and visit documentation for accuracy, completeness, and compliance Drive timely physician order turnaround (tracking, follow-up, escalation when needed) Identify and resolve documentation issues proactively — not reactively Ensure clinicians are aligned with plan of care, frequency, and utilization Monitor case progression and intervene early to prevent delays or compliance risks Support intake and authorization teams with clinical decision-making as needed Maintain audit-ready charts at all times Participate in case conferences with a focus on outcomes and efficiency Act as a resource and escalation point for clinical and documentation issues What Success Looks Like:
Support timely patient care through efficient clinical review and decision-making Maintain complete, accurate, and compliant patient charts Ensure timely OASIS review, physician orders, and documentation turnaround Collaborate with clinicians to support smooth case flow and continuity of care Manage high volume while maintaining quality, compliance, and timeliness Join a collaborative clinical team where your expertise is valued and your work directly impacts patient outcomes and operational excellence.
