Job Description

West County Health Centers (WCHC) provides comprehensive and quality health care services to the communities of Western Sonoma County. We are a cohesive team of health care providers, support staff and volunteers dedicated to wellness, affordability, and excellence of care. Come and be a part of an amazing team! The Substance Use Disorders Services (SUDS) Program Contractor will be responsible for the development of the Substance Use Disorder Services Program. SUDS Program ensures that the goals and objectives regarding access to quality medical and Mental Health services and projects related to these services within the context of substance use disorders are provided to patients in need of treatment. Interventions include medication assisted treatment in primary care, treatment for substance use disorders, and case management services to meet their health and mental health needs. The SUDS Program contractor will write, maintain, and updates policies and procedures, patient-facing educational materials, and other administrative documentation necessary to implement SUD treatment and related services. At the direction of the MAT medical and MH leadership, SUDS Senior Program consultant is responsible for the work necessary to make these documents available agency wide and support training to relevant teams and team members to understand and implement the written material. WCHC provides Enhanced Care Management (ECM) services under CalAIM to people needing extra support to resolve social determinants of health, and this includes support for people with substance use disorders.
Deliverables:
Develop, organize and implements program requirements defined by WCHC, Sonoma County and other partner agencies. Oversees and manages the day-to-day operations of SUD treatment continuum: including but not limited to ASAM assessments, creation of treatment plans, access to and engagement with: individual and group treatment, MAT services, case management services, nursing services. Oversees the general operations of Enhanced Care Management (ECM) services Facilitates required documentation in multiple medical record systems as required by program and contract agreements. Oversees data collection with the purpose of facilitating data-driven evidence of program successes and areas of growth, and for grant and contract reporting including overseeing data entry and participation in the County's medical record system. Completes and submits data and documentation reports as defined by program and contract agreements. Represents WCHC programs at community health care and drug and alcohol agencies. Participates in community outreach events on behalf of WCHC Creates, keeps track of, and evaluates MOUs and contracts with allied agencies. Provides outreach regarding SUDS to all WCHC programs and services. Maintains expertise on subject matter by staying current with rehabilitation tools and information related to substance use treatment, researching health trends, policies and statistics related to these topics.
POSITION REQUIREMENTS
Two to three years of experience running a program that integrates multidisciplinary teams and with multiple agencies including federally qualified health centers, social service agencies, therapy providers, AOD service providers, health plans, and County departments. BA or BS in Health Education, Addiction or AODs, Social Work, or related field desired or like experience Trained as Case Manager, Community Health Work, Social Worker, or AOD Counselor highly desired Experience developing effective workflows that support systems and department goals Experience with substance use education, navigation or treatment desired Experience working with at-risk populations of all backgrounds in settings or in agencies providing similar services Experience in a community health center setting desired Ability to work well with a multidisciplinary team and autonomously Ability to problem solve and be self-motivated with attention to detail Must have excellent written and oral skills, strong organizational and analytical skills Must have ability to manage priorities and workflow Expected to have strong interpersonal skills and ability to work well with individuals at all levels of the organization.
