This is an excellent opportunity for a compassionate social worker with skilled nursing or hospital experience who enjoys helping patients navigate the healthcare system and connect with essential services. Position Responsibilities Complete comprehensive health risk assessments Develop individualized, person-centered care plans based on each member's needs and goals Coordinate care with primary care providers, specialists, and community resources Conduct bedside visits for members during hospitalizations Maintain accurate, timely documentation in compliance with organizational and regulatory standards Advocate for members to ensure continuity of care and access to appropriate services Qualifications Master's Degree in Social Work (MSW) Active ACSW registration in California Previous hospital or skilled nursing experience required Strong assessment, communication, and care coordination skills Ability to work independently and manage multiple priorities Commitment to culturally responsive, person-centered care Bilingual Spanish is a plus Compensation & Schedule $36/hour Monday-Friday, 8:30 AM-5:00 PM 100% on-site in Los Angeles, CA 90027 1-6 month W-2 assignment Apply Today If you're interested in this opportunity, please apply online or email your resume to .
