Case Manager Confidential Malibu, CA Job Details Full-time $24 - $26 an hour 4 days ago Benefits Health insurance Dental insurance Paid time off Retirement plan Qualifications Microsoft Excel Driver's License Case management Clinical information systems Discharge planning Productivity software Certified Case Manager Documentation review Technical Proficiency Full Job Description Responsibilities for Case Manager Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation Work with clients with discharge planning, finding meetings, supporting throughout their treatment Communicate patient needs to a variety of care team members and follow up accordingly Manage discharge plans upon completion of treatment Work collaboratively with patients, families, physicians, and nurses to ensure a high standard of care Act as the patient's advocate as it relates to insurance coverage and financial assistance Maintain the patient's comprehensive clinical record through detailed documentation Coordinate with third party payors on a regular basis Set up scheduling of patient meetings, events, and appointments at all stages of treatment Ability to handle case load of 10-15 patients at a time Ability to work interdepartmentally and cohesively with both internal and external providers Knowledge of medical billing process a plus Qualifications for Case Manager Bachelor's degree in Social Work or Psychology Certified Case Manager (preferred but not necessary) 1-2 years of clinical experience, preferably in case management String knowledge of EMR software Proficient computer skills including experience with Microsoft programs Strong written and verbal communication skills Excellent clinical assessment and analytical skills
