Job Description

Position:
Clinical Case Manager II Salary:
$32.92 per hour (Non-Exempt) with employer-covered health benefits Reports to: Clinical Program Director II or III Conard House, Inc. is a nonprofit organization empowering people and restoring hope. We work with a diverse adult population and strive for diversity by respecting experience, promoting inclusion and building community. Conard House Inc. contracts with the San Francisco Department of Public Health and Department of Human Services to provide a cohesive array of support services to a client population.
Qualifications:
Bachelor's Degree required. 4 years of experience working in a mental health setting required. At least one year of experience supervising staff and managing a program, demonstrating effective leadership skills. Must demonstrate case management skills and a thorough knowledge of mental health interventions, terminology, and experience with outpatient services billing. Effective assessment, advocacy, organizational, and communication skills required. Experience working with diverse populations and within a diverse team. Experience with Medi-Cal Billing and EPIC database strongly preferred. Knowledge of Supported Self-Management Model, Strength-Based, and Recovery-Based services a plus. Must successfully complete a background and reference review.
Responsibilities:
Assist residents in managing bills, purchases, and other financial matters. Offer a wide range of resources to clients, addressing problems in a supportive, constructive, and non-judgmental manner. Admit and orient new clients to the program. Assess clients' needs for medical and psychiatric evaluations, making appropriate referrals. Develop and implement case management and money management plans and contracts. Coordinate Plans of Care with other services being provided to the client. Advocate for clients in securing entitlements, clothing, healthcare, mental health services, and other needs. Ensure client applications and forms are completed accurately. Lead groups and activities focused on stable living, promoting interaction among clients and engagement with social programs. Meet monthly billing targets. Create and maintain a supportive, independent living environment. Foster community building within the hotel and assist clients in accessing community resources. Respond to crisis situations in accordance with established procedures. Support clients in developing daily living skills. Plan and implement strategies for client placement upon discharge from the program.

Schedule:
Monday to Friday, onsite 9am to 5pm 1 hour paid lunch break Conard House is committed to workforce diversity.
