JOB SUMMARY
This role manages a team of case managers providing community-based care to Medi-Cal members with complex physical, behavioral health, and social needs under California's CalAIM initiative. This role manages workflows and performance, ensures compliance with DHCS and Managed Care Plan requirements, and monitors caseloads, member acuity, and key service metrics. This role also coordinates care with Managed Care Plans and other programs, helps connect clients to appropriate community resources, reviews and audits documentation for quality and accuracy, and supports continuous improvement in service delivery and client outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES
? Oversee daily operations of the ECM Program ensuring compliance with CalAIM Managed Care Plan requirements and organizational standards. ? Create and manage team schedules. ? Identify and apply career advancement opportunities to employees under their direction. ? Maintain a small client caseload. ? Attend all necessary meetings. ? Practice and reinforce the mission, vision and values of the organization. ? Recruit, interview, hire, and orient all team members. ? Maintain appropriate staffing levels and caseload distribution. ? Provide initial and ongoing training to team members. ? Assign duties and work schedules to team members and provides on-site guidance of established procedures ? Review and approve timecards, including time off requests. Make appropriate edits. ? Monitor overtime, doubletime, and missed meal breaks. If abuse is apparent, meet with employee(s) to discuss. If abuse continues, work with HR for disciplinary actions. ? Maintain job descriptions and evaluation criteria for all direct reports, using the appropriate tools provided by HR. ? Provide ongoing individual and group supervision, coaching, performance evaluations and assist with any questions or problems. ? Address any performance problems and, if necessary, handle terminations in accordance with established company guidelines. ? Address performance issues and implement corrective actions in collaboration with HR. ? Maintain OSHA standards. ? Responsible for the collection of data and reporting via various software programs. ? Hold and facilitate regular team meetings. ? Oversee program budget and ensure that projects are completed on time and in budget. ? Ensure all ECM documentation meets CalAIM Managed Care Plan monitoring requirements and ensure readiness for audit. ? Assist in the completion of grant applications, RFP's and RFQ's. ? Provide individual supervision to team. ? Provide group supervision to staff, trainees, and associates. ? Review and approve client records including assessments, progress notes, care plans, and discharge summaries for accuracy and compliance. ? Monitor client progress, outcomes, and service engagement, and ensure timely intervention when needed. ? Oversee high-risk cases, including risk assessment, incident response, and coordination with external providers and agencies. ? Supervise program evaluation and develop QIP's. ? Perform other duties as assigned.
JOB REQUIREMENTS
A Master degree in Social Work, Gerontology, or a related field, preferred. Bachelors' degree in related field required. A minimum of five years' experience in direct service with older adults as a case management or medical social worker. A minimum of 1year Supervisory experience. Experience overseeing compliance audits. Substance abuse and/or dual diagnosis training and experience. Experience handling crisis situations. Experience working with homeless or disenfranchised older adult clients. Experience working with individuals and groups. Experience working with older adults in cognitive decline. Demonstrated commitment to diversity, equity, and inclusion. Bilingual/bicultural a plus. Proof of negative or inactive TB required. Curry Senior Center will provide or reimburse for the cost of a TB test after an offer of employment has been accepted by both parties. Proof of a negative TB test within 90 days will also be accepted.
KNOWLEDGE, SKILL, AND ABILITIES
Effective oral and written communications skills with the ability to express ideas in a clear, thorough, concise manner. Strong interpersonal, communication and organizational skills. Solid aptitude for customer service. High degree of skill in grammar, spelling and punctuation. Ability to maintain the confidentiality of all information obtained. Strong organizational skills and the ability to prioritize workflow. High level of analytical problem-solving skills. Excellent time management skills, including the ability to manage self-development. Must be action-oriented and have the ability to work independently and as a team player. Strong computer skills such as Chrome, Google suite, Apple iOS, Office suite.
PHYSICAL REQUIREMENTS/DEMANDS
The incumbent is in a non-confined office-type setting in which they are free to move about at will. The Environment is mostly clean and comfortable but may include some minor annoyances such as noise, odors, drafts, etc. Work will be performed utilizing a computer, sitting at a desk/chair or standing while speaking and interacting with clients. Position will require average finger dexterity to operate normal office equipment and systems.
