  • Full-time Regional Clinical Liaison Southern Colorado Region Full Time | Competitive Pay and Benefits We are seeking a dedicated Regional Clinical Liaison to support multiple skilled nursing facilities throughout Southern Colorado.
This role serves as the clinical bridge between hospitals, physicians, case managers, discharge planners, patients, families, and our facilities to ensure smooth transitions of care and exceptional patient outcomes. The ideal candidate has experience in skilled nursing, case management, admissions, discharge planning, healthcare marketing, or hospital liaison services and is passionate about building strong relationships while advocating for quality patient care.

Responsibilities:
Develop and maintain relationships with hospitals, physicians, case managers, social workers, and referral sources Assess and coordinate appropriate patient referrals for skilled nursing and rehabilitation services Facilitate smooth transitions from acute care settings to post-acute care facilities Educate patients, families, and healthcare professionals regarding available rehabilitation and skilled nursing services Collaborate with facility clinical and admissions teams to ensure timely admissions and continuity of care Travel throughout the region to hospitals, healthcare providers, and supported facilities Participate in care transition meetings, discharge planning discussions, and community outreach activities Serve as a clinical resource and advocate for patients, families, and referral partners
Qualifications:
Current RN, LPN, PTA, OTA, Social Work, Case Management, or related healthcare experience preferred Previous experience in skilled nursing, rehabilitation, hospital discharge planning, admissions, or clinical liaison services preferred Strong understanding of post-acute care services and patient transition processes Excellent communication and relationship-building skills Self-motivated with the ability to work independently Reliable transportation required Ability to travel throughout Southern Colorado Apply Today Join a growing healthcare team committed to providing exceptional care, building trusted community partnerships, and supporting patients through every step of their recovery journey.
