Job Description

Summary:
The Medical Case Manager assesses client needs, develops a service plan with the client, helps the client negotiate the medical and support service systems and makes appropriate linkages to needed services, monitors client progress and acts as an advocate for the client whenever necessary. The medical case manager provides these services in a pro-active manner with the client, utilizing all community resources.
Responsibilities:
Provide comprehensive initial and ongoing assessment of the client's needs and personal support systems. Develop an individualized service plan. Provide timely, coordinated access to medically appropriate levels of health and support services required to implement the service plan. Assist clients establish and maintain linkages with community resources (health care, psychosocial, financial, housing, legal), including direct advocacy and/or assistance in accessing these services. Assist clients secure and maintain entitlements; ensure clients' rights, welfare entitlements, and health and social services have been provided in a just and respectful manner. Monitor, evaluate, and revise the client service plan as necessary according to the adequacy of services/interventions in meeting the client needs. Maintain appropriate working relationships with the client and family. Maintain effective working relationships with members of the Greater Bridgeport
HIV/AIDS
Care Consortium and all community resources, which are significant to the client's well being/service plan. Identify and report to the program supervisor any problems with the service delivery systems that require advocacy. Attend scheduled team meetings, clinical supervision, and monthly consortium meetings. Attend a minimum of 21 education credit hours of HIV-specific training per year. Organize workload so as to meet productivity expectations while maintaining an appropriate level of service to the client. Learn and follow program/agency policies; clearly interprets program to client, colleagues and others in the community. Keep accurate and timely confidential client records. Other related duties as assigned.
Qualification:
Associate degree in human services or equivalent. 5 years case management experience. Ability and desire to work with
HIV/AIDS
clients and their families. Sensitivity to
HIV/AIDS
related issues, such as stigma, culture, and dual diagnosis of mental health and/or substance abuse. Knowledge of community medical and social service systems Exceptional interpersonal skills. Good oral and written communication skills. Must be able to communicate and establish rapport with target population and with community partners. Ability to keep accurate records and maintain confidentiality. Computer literacy including word processing and basic database entry. Current CT driver's license, own transportation and proof of insurance and registration. Bilingual English\Spanish is benificial.
Work Environment/Physical Demands:
The work environment will include home visits in potential high-risk areas, visits with community partners, in addition to the typical office setting. The position requires driving, walking, standing, bending and some lifting. The work environment involves the normal risks associated with visits to housing projects, home visits and high-risk areas, as well as discomforts typical of a community based organization.
Pay:
$22.00 - $25.00 per hour
