Full Job Description Case Manager/MDS Coordinator Job Summary:
At Vernon Rehabilitation and Healthcare Center, we are committed to delivering exceptional care to our residents. As a Case Manager/MDS Coordinator, you will play a crucial role in ensuring all residents have continuation of stay interacting with representatives of managed care companies. We are seeking a dedicated and organized professional to join our team on a full-time basis.Responsibilities:
Liaison with management, nursing, social services and admissions for case management determination. Manage Medicare Notice of Non Coverage process including, but not limited to staying current with regulatory changes, timely notification of patients/responsible party, instructing facility staff in the required tasks. Complete managed care assessment reports. Maintains managed care contracts and ensures facility adherence to contractual stipulations in the delivery of care. Ensures managed levels of care align with patient's individualized plan of care and seeks to reconcile noted expectations (i.e., high cost pharmaceuticals, wound vacs, etc.) Coordinate and facilitate interdisciplinary team meetings to ensure comprehensive care Complete and maintain accurate MDS (Minimum Data Set) assessments and care plans Participates in 72-hour meetings and collaborates with Social Services pertaining to discharge planning. Document patient data and maintain accurate recordsRequirements:
State of CT Licensed Practical Nurse (LPN) in good standing 1 year minimum experience with MDS (Minimum Data Set) 2 years of experience in a long-term or post-acute settingWork Environment:
40 hours per week, Monday through Friday with some flexibility on the day shift Vernon, Connecticut locationAbout Us:
