The range of services includes accessing medical, social, community, legal, financial, employment, vocational, and/or other needed services. Will also assist eligible clients to obtain access to other public and private programs for which they may be eligible. NMCM Services includes all types of case management encounters (e.g., face-to-face, phone contact, and any other forms of communication). NMCM will manage a shared caseload of clients, submit monthly and quarterly grant reports, and conduct routine, conduct client data entry and reporting. The NMCM works directly with women living with HIV/AIDS, to support, educate and assist them in accessing medical, psychological/psychosocial, and other support services that will positively affect their overall wellness, mental health and medical health outcomes as, well as their ability to remain in care and treatment. This position includes the coordination, documentation and tracking of referrals and services provided, and facilitation of support groups which are delivered by all members of the Care Team. Duties and responsibilities- Support the client base for TWC's case management system. Facilitate access to intake screening and follow-up interviews for clients/family members/partner(s) in person, over the telephone, in hospital or care settings.
- Practice person-centered planning; integrate into the creation of the service planning process with the primary focus on connection to and retention in services.
- Ongoing assessment of the client's and other key family members' needs and personal support systems
- Ensure that case plans and other records are complete, updated, and accurate; including timely completion of assessments
- Assess clients' medical and bio-psychosocial needs including, but not limited to, medical care, mental health care, treatment adherence support, housing, psychosocial support, home health care, respite care, legal services, employment training, entitlements, food, substance abuse treatment, and counseling, etc.
- Assist clients in finding housing and/or make referrals to housing assistance agencies.
- Develop strength-based service plans including referrals to appropriate service providers, community-based organizations, and entitlements for the client, family members, and/or partner(s).
- Support coordination of treatment adherence program among care team members
- Provide HIV risk reduction counseling to support client behavior change.
- Provide individual counseling, behavior modification techniques, case management and crisis intervention as needed
- Conduct ongoing monitoring including regular reassessment, care plan modifications, referrals, and recommendations to ensure client progress is appropriate.
- Maintain clear, accurate, up-to-date documentation on all clients in an ethical and confidential manner including in hard copy files and CARE Ware (our client database).
- Complete daily contact notes within 24 hours and ensure all documentation contains descriptive, analytical, and evaluative content.
- Practice cultural sensitivity and responsiveness in all day-to- day interactions.
- Advocate for clients in voicing complaints, concerns, and grievances when appropriate. Facilitate or coordinate mediation services when necessary.
- Provide monthly, quarterly, and annual reports in a timely fashion that include, but not limited to, summaries of services provided, barriers experienced, and success stories.
- Assist in organizing client training and events and maintaining the TWC kitchen pantry.
- Work as an integral and participating member of the Care Team; facilitate a continuum of care models through partnership and collaboration with all TWC programs.
- Work with Programs Manager to improve the delivery of services to clients.
- Participate in all staff, care team, and community meetings as scheduled.
- Perform other duties as assigned to meet clients' needs, funder requirements, and agency-wide initiatives. Qualifications
Education:
Minimum of Bachelor's degree in health or human services field preferredCase Management:
Minimum of 2 years' experience working with persons living with HIV or at high risk of HIV preferred. Experience in the field of case management and treatment adherence interventions with people living with HIV preferred.- Writing and analytical skills: Clear, professional, and detail-oriented computer, writing and documentation skills and experience with data collection and reporting.
