Job Description

East Ridge Village is seeking a Case Manager. The Case Manager is responsible for preparing assessments for our residents at our facility to ensure the services they receive are appropriate and effective. QUALIFICATIONS
MDS coordinator certification Clinical Experience in a nursing facility Demonstrated clinical assessment skills Knowledge of Medicare and Medicaid regulations and coverage guidelines as related to skilled nursing.
