Peer Recovery Case Manager Serenus Detox Jacksonville, FL Job Details Full-time $45,000 - $50,000 a year 20 hours ago Qualifications Computer operation CPR Certification Computer literacy Intake High school diploma or GED First Aid Certification Case management Substance abuse Clinical documentation Computer skills Full Job Description Position Summary The Peer Support Specialist/Case Manager provides recovery-oriented support and care coordination for individuals participating in an ambulatory detoxification program. This role combines lived experience, peer engagement, and case management services to help clients navigate substance use recovery, access community resources, maintain treatment participation, and develop individualized recovery plans. The position works collaboratively with clinical staff, medical providers, and community partners to support client stability, safety, and long-term recovery outcomes. Key Responsibilities Peer Support Services Provide non-clinical peer support to individuals in recovery from substance use disorders. Use lived experience to build trust, encourage engagement, and promote hope and motivation for recovery. Assist clients in identifying personal recovery goals and wellness strategies. Facilitate recovery-focused groups and peer support activities. Support clients in developing coping skills, relapse prevention strategies, and healthy routines. Encourage participation in treatment, community recovery programs, and support networks. Case Management Conduct intake screenings and assist with needs assessments. Develop and monitor individualized service and recovery plans. Coordinate referrals for medical care, mental health treatment, housing, employment, transportation, food assistance, and other community resources. Assist clients with appointment scheduling, benefits applications, and service navigation. Maintain regular follow-up contact with clients to assess progress and barriers to care. Advocate for client needs within the healthcare and social service systems. Ambulatory Detox Support Monitor client participation and engagement throughout the detoxification process. Observe and report concerns related to withdrawal symptoms, relapse risk, or safety issues to clinical staff. Reinforce education provided by medical and clinical teams regarding medication adherence, treatment expectations, and recovery planning. Assist with discharge planning and transition to ongoing levels of care, including outpatient treatment, residential treatment, MAT programs, or recovery housing. Documentation & Compliance Complete timely and accurate documentation in the electronic health record. Maintain confidentiality in accordance with HIPAA and organizational policies. Participate in multidisciplinary team meetings and case consultations. Maintain required certifications and training standards. QualificationsRequired High school diploma or GED. Certified Peer Specialist, Peer Recovery Coach, or equivalent state-recognized peer credential (or ability to obtain within required timeframe). Experience working with individuals affected by substance use disorders and/or co-occurring mental health conditions. Strong communication, organizational, and interpersonal skills. Ability to work effectively with diverse populations. Basic computer and documentation skills. Preferred Associate or bachelor's degree in social work, psychology, counseling, human services, or related field. Previous experience in detoxification, behavioral health, substance use treatment, or community health settings. Knowledge of local recovery and community resource systems. Case management experience. CPR/First Aid certification.
