Job Description

Are you looking for a HIV medical case manager position familiar with Ryan White services to enhance the quality of client care and services in accordance with the guidelines established by the Health Resource Service Administration, Bureau of
HIV/AIDS
; and the Florida Department of Health? Case Management duties include assessing, planning, implementing, monitoring, evaluating actions and observing strict confidentiality required to meet the client's health and human services needed. ESSENTIAL FUNCTIONS
Major Responsibilities and Duties:
Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered, removing barriers to care. Handle a minimum case load of 60 assigned clients, Draft, monitor and updateservice plans, review case progress and determine case closures as necessary Help clients achieve autonomy by promoting health literacy and wholistic wellness Facilitate multiple care aspects (case coordination, information sharing, etc.) Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options Develop effective working relations and cooperate with internal and external stakeholders throughout the entire case management process Ability to assess overall workflow to promote alignment with Federal, State, Local and Agency Key Performance Indicators (KPIs) Coordinate care with other social service, health and governmental agencies Maintain required monthly, quarterly and annual client encounters required by the Program Document detailed and accurate case information in required electronic health records and other required data management platforms Complete all required forms to maintain client enrollment and program eligibility Promote quality interventions and outcomes Ensure that any applicable hierarch of payer sources is used appropriately Advocating and upholding program policies and procedures Assess and address motivational and psychosocial issues Adhere to professional standards as outlined by protocols, rules and regulations Builds knowledge of HIV disease, it's treatments and services Help to remove stigma and promote wellness and education.
QUALIFICATIONS
Bachelor's degree in a social service, or health field OR Bachelor's degree in a human science such as psychology, health, sociology, biology, human development and anthropology OR Registered Nurse without a Bachelor's degree who has a minimum of one year case management experience; OR Licensed Practical Nurse without Bachelor's degree who has a minimum of three years case management experience. Proficient in Microsoft 365 Suite eClinicalWorks or other EMR software LGBTQ+ and those with Lived Experience encouraged to apply Bilingual a plus, Spanish, Creole, and/or ASL All applicants must pass a

$52,500.00 - $55,000.00 per year
