The Bilingual Medical Case Manager contributes to AHF by utilizing their experience, skills, and education together with a thorough understanding of the Ryan White Care Act Program within their specific area. With a caring and knowledgeable focus, you will work with the immigrant community and new arrivals so that every Ryan White client receives the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood. Working directly with CALOR, you will meet with clients and complete Needs Assessments, develop Action/Care Plans, and provide ongoing education to patients so that they understand their diagnoses and their medications. For Ryan White contracts that combine psychosocial needs within Medical Care Management, you will understand mental health needs and basic needs such as housing, food, and clothing and develop relationships with programs and agencies serving those needs and facilitate referrals, support, and assistance for your patients who lack those services. The Bilingual Medical Case Manager enhances the reputation of AHF by utilizing your case management experience, you will meet clients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. AHF has a collaborative organizational structure where staff are accountable to multiple leaders. The Ryan White Bilingual Medical Case Manager will work as a part of a close-knit team to be the liaison between provider and client and can facilitate the shift from leaving the exam room to implementing the optimum plan of care and putting it into action. Providing brief interventions focused on a long-term achievable goal, the client and the MCM work together each step of the way towards viral suppression, better health outcomes and quality of life.

Bachelor's or master's degree in Social Science, Social Work, or Human Services. Fully fluent/bilingual in Spanish is required. A minimum of 1 year of Case Management experience is preferred.

Knowledge of Ryan White Services is preferable.
