Job Description

Job Summary We are seeking a dedicated and energetic Qualified Intellectual Disabilities Professional (QIDP)/Case Manager to join our dynamic team. In this vital role, you will coordinate and oversee comprehensive care plans for individuals with intellectual and developmental disabilities, ensuring they receive personalized support that promotes independence, community integration, and quality of life. Your expertise will drive effective case management, facilitate access to services, and advocate for the needs of each individual. This position offers an exciting opportunity to make a meaningful difference in the lives of those we serve while working within a collaborative healthcare environment committed to excellence. The QIDP is responsible for coordinating, integrating, and monitoring active treatment and support services for individuals with intellectual and developmental disabilities. You will serve as the primary advocate and case manager, ensuring that Individual Service Plans (ISPs) are developed and implemented with a person-centered focus.
Responsibilities Service Planning :
Facilitate Interdisciplinary Team (IDT) meetings to develop, implement, and regularly review ISPs and behavior support plans.
Documentation & Compliance :
Maintain comprehensive clinical records in accordance with Illinois Department of Human Services (IDHS) regulations.
Program Monitoring :
Review data collection and progress notes to ensure that services are being delivered as outlined in the ISP.
Crisis Intervention :
Provide support and guidance during medical or behavioral crises, ensuring physical safety and appropriate follow-up care.
Communication :
Serve as liaison for assigned caseload with needed services such as day programs, transportation etc.
Case Management & Advocacy :
Act as the primary lead for an assigned caseload, advocating for the rights, dignity, and personal goals of each individual
Pay:
$60,000.00 - $80,000.00 per year
