SUMMARY:

The case manager is the resource person in the area of inpatient case management and discharge planning.

DUTIES PERFORMED

Conducts psychosocial history to determine pre-injury issues that need to be addressed. Establishes and carries out an individualized treatment plan upon completion of psychosocial history, if applicable. Provides counseling to clients, families, and/or significant others. Develops and implements family and children support groups as needed. Schedules, arranges, and moderates the program for family education groups. Participates in team rounds and family conferences as scheduled through the case manager. Maintains records and prepares reports according to facility procedures. Participates in and documents internal and external professional educational activities to enhance personal and professional growth. Performs public relations tasks as directed by supervisor. Coordinates the client's schedule with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialists, and other disciplines as needed. Is an integral part of the interdisciplinary team. Key component in assisting in patient discharge planning. Conducts post discharge follow-up as needed. Any other tasks as assigned by the immediate supervisor.

SPECIFICATIONS

Bachelor's in Social Work or related field

Preferred Formal Education/Formal Training:

Degree/license in healthcare

Preferred Job-Related Experience:

Minimum of two years' experience in a hospital/health care setting, preferably in an Inpatient Rehab Facility (IRF)
