As a licensed AODE and DUI treatment facility, we provide outpatient programs including counseling, intensive outpatient programming, medication-assisted treatment, and case management. Position:
CommonHealth Recovery is seeking a dedicated and organized Targeted Case Manager to assist clients in achieving recovery by addressing non-clinical barriers that affect treatment engagement. The ideal candidate will provide high-quality support services, including resource coordination, care planning, and advocacy. This role is essential in helping clients meet their basic needs while participating in treatment.
Work Environment:
You'll be part of a collaborative, multidisciplinary team of counselors, medical providers, and administrators working toward client stabilization and long-term recovery. The environment is a balance of structured documentation, dynamic client interactions, and team collaboration.
Key Responsibilities:
Provide case management services for individuals with substance use and co-occurring mental health disorders. Complete initial and ongoing client assessments related to housing, transportation, employment, medical needs, and other social determinants of health. Develop and monitor individualized care plans based on client needs, goals, and treatment recommendations. Refer clients to community-based services and coordinate with agencies to ensure access to resources. Document all client interactions, assessments, and progress in the electronic medical record (EMR) in a timely and professional manner. Communicate regularly with clinical and medical teams to support treatment coordination. Participate in treatment team meetings and case consultations. Maintain client confidentiality and comply with HIPAA and Medicaid documentation requirements. Support clients with navigating benefits such as SNAP, Medicaid renewals, housing applications, and more. Provide crisis intervention and support clients in identifying and addressing barriers to treatment engagement. Assist with appointment scheduling and reminder calls as needed to support client retention. Other duties as assigned by supervisor or leadership.
Qualifications:
Required:
Bachelor's degree in Social Work, Psychology, Counseling, or related field.
Preferred:
Kentucky state certification as a Targeted Case Manager or willingness to complete TCM training upon hire. Experience working with vulnerable or high-risk populations preferred. Strong verbal and written communication skills, documentation proficiency, and organization. Demonstrated reliability and punctuality. Ability to work with diverse populations and maintain cultural humility. If in recovery, must have at least one year of continuous sobriety and active participation in a recovery program.
