Job Description

Targeted Case Manager Liberty Place Recovery Center Mountain Comprehensive Care Center Mountain Comprehensive Care Center is hiring a Targeted Case Manager to serve clients at Liberty Place Recovery Center . The Targeted Case Manager provides supportive case management services to individuals actively participating in treatment by helping connect them with resources, coordinating care, and address barriers that may impact recovery, stability, and overall quality of life. This position requires strong verbal and written communication skills, sound clinical judgment, the ability to work independently, and a commitment to providing quality, team-based care. Primary Responsibilities The Targeted Case Manager will: Assess client needs, strengths, level of functioning, support needs, risk factors, and barriers to care. Gather relevant information from clients, treatment team members, medical providers, social service agencies, family supports, and other collateral contacts as appropriate. Develop individualized service plans that reflect the client's goals, needs, strengths, and recovery vision. Coordinate referrals and link clients with appropriate community resources, benefits, medical care, housing supports, employment resources, transportation, and other needed services. Provide direct and indirect case management services to support client progress and continuity of care. Document client contacts, services provided, referrals, coordination efforts, client response, and progress toward service plan goals. Communicate relevant client needs, barriers, and progress with clinical staff and supervisors. Monitor care plans, progress notes, service activities, and client outcomes in collaboration with the clinical supervisor. Complete prior authorizations and related documentation as needed. Attend care plan meetings, treatment team meetings, and other required program meetings. Maintain an active caseload of approximately 25 clients. Maintain documentation in accordance with agency, state, federal, and payer requirements. Qualifications Minimum qualifications include a Bachelor's degree in a human services field , such as Psychology, Social Work, Counseling, or a related area, and at least one year of case management experience . A Master's degree in a human services field may substitute for the required case management experience.
