Job Description
Family Road of Greater Baton Rouge is seeking a Maternal and Child Health Case Management Supervisor for the Healthy Start Program. This position will manage and supervise the daily operational activities of the case management team, clinical support and training, provide case supervision to ensure home visits, charting, database entry and follow-ups are conducted, provide oversight of program data collection/evaluation and generate data outcome reports, conduct quality assurance case reviews and assist the Program Director and Management Team in the writing of all grants and grant performance reports. The Case Management Supervisor position requires 1.) A master's degree in social work. (candidate must be a Licensed Clinical Social Worker (LCSW) or currently pursuing licensure) and the candidate must have 2.) A minimum of five (5) years post-master's degree social work experience with two (2) of the five (5) years experience being at a management/supervisory level.
