Job Description

We are seeking a dedicated Community Care Provider to join our team. Reporting to the Clinical Manager, you will be responsible for providing strength-based, solution-focused therapeutic services in the community. Our company prides itself on delivering consumer-driven and family-oriented services, conducted within the context of the consumers natural environment. We are looking for a candidate who is knowledgeable, adaptable, and committed to providing the highest quality of care to our clients. Responsibilities Provide strength-based, solution-focused supportive therapeutic services in the community. Deliver client-centered, individual Psychosocial Skills Rehabilitation (PSR) services in the community. Assess each recipient for Strengths, Resources, and Goals within first 30 days of treatment; provide Strength-based planning and treatment on an ongoing basis. Participate in collaborative meetings with Rehabilitation Services staff, other agencies or community and natural supports as needed. Provide Crisis Intervention services to recipients as needed according to agency policies, procedures, and service definitions. Communicate clients status on a regular basis, including changes in mood or functioning, medical issues and all crisis and health and safety issues. Produce effective, clinical documentation of every contact with recipient. Assist with the completion of 90 Day client reviews Attend all office trainings as well as regular clinical supervision sessions with Clinical Manager Return phone calls promptly and follow all safety rules. Complete all yearly trainings Qualifications Masters Degree from an accredited university or college in the field of Counseling, Social Work, Psychology or Sociology. Ability to use computer interface to complete documentation for billing or other purposes as needed. Excellent communication and documentation skills. Ability to work collaboratively with a diverse team.
Offered compensation:
$20/hr - $23/hr Experience and License Requirements Master's degree in Counseling, Social Work, Psychology or Sociology
