Commonwealth Land Trust Medical Case Manager Location:
multiple sites in Boston, MA - in the Dorchester area or
Beacon Hill SUMMARY:
Commonwealth Land Trust provides permanent supportive housing and social services to individuals who have experienced chronic homelessness, substance addiction, and physical and mental health conditions. The Medical Case Manager provides case management services, addictions relapse prevention, mental health and
HIV/AIDS
health concerns support, crisis response, and educational groups to clients. Case managers provide services on-site where the clients reside. Bi-lingual Spanish Preferred.
COMPENSATION FOR THIS POSITION
$55,000 - $65,000 DOE with benefits. Full-time employees receive health, dental and life insurance, and generous time off.
DUTIES INCLUDE, BUT ARE NOT LIMITED TO
  • Conducting initial client intakes and assessment interviews
  • Designing individual service plans for each client tailored to their needs
  • Collaborating with outside providers and case managers to ensure delivery of needed services to our residents
  • Identifying addictions and mental health concerns among clients and offering appropriate support
  • Recording accurate case notes and maintaining all documentation in accordance with funder requirements
  • Coordinating and leading educational groups with clients on addiction, HIV/AIDS, and other health issues
  • Provide crisis intervention and de-escalation as needed
  • Thorough documentation of services utilizing an electronic data base
QUALIFICATIONS
The ideal candidate will be:
  • Well-versed in addictions-related issues (and preferably have a CDAC or equivalent training/work experience), comfortable with a harm-reduction approach to clients, and know how to deescalate and manage situations appropriately.
  • Experienced and trained in mental health, social service provision and/or case management, and/or with prior experience working with populations similar to those we serve, including people with HIV, people struggling with drug and alcohol addiction, and people living with mental illness.
  • Able to work well independently, as well as with a team that includes supportive housing managers, staff assistants, outside social service providers and community groups.
  • Experienced with Microsoft Office programs and skilled in clinical documentation.
  • Bilingual (Spanish & English) both written and spoken, preferred.
  • Physical ability to climb stairs
