Job Description

The Center for Human Development, (CHD) is seeking an experienced and creative Case Manager at its Two Rivers Recovery Center in Greenfield. Two Rivers is a program for women in early recovery from substance use who are pregnant or postpartum mothers with their infants. The program supports up to 25 women and 5 infants in long term (6 to 12 months) residential treatment. The comprehensive clinical service is a trauma informed, harm-reduction model. Your role as a
Case Manager:
The Case Managers is responsible for the development and implementation of person-centered treatment plans, facilitating evidenced-based therapeutic group, individual therapy, assistance with discharge planning, as well as community support coordination and collaboration. The Case Manager receives supervision from a professional, experienced Clinical Director.
Requirements:
High school diploma or equivalent required Bachelor's level required Knowledge of substance use disorders and treatment Experience facilitating groups Must pass background screening process including CORI Valid driver's license and have a clear driving record check Have reliable transportation, for occasional business use

Saturday Starting Salary:
