The Latin American Health Alliance (LAHA) is a dedicated non-profit organization based in Central Massachusetts, focused on improving the health and well-being of underserved communities, particularly Latinx population. We provide culturally sensitive addiction recovery services, healthcare access, and educational support. Our mission is to foster long-term health, stability, and empowerment, promoting sustainable solutions for individuals facing significant challenges related to substance use disorders. Position Summary The Case Manager plays a fundamental role in supporting residents through their recovery journey by working collaboratively with the clinical team, medical staff, and external resources. This role involves delivering resident-centered services, including individualized support, group facilitation, treatment planning, discharge coordination, and aftercare planning. The Case Manager ensures that patients are empowered to achieve their recovery and life goals while maintaining the highest standards of care and professionalism. Essential Duties and Responsibilities The essential functions include, but are not limited to, the following: Foster a collaborative, residentcentered relationship by reducing traditional power differentials. Act as an integral member of a multidisciplinary team delivering comprehensive recovery and treatment programming. Partner with residents, clinical staff, and medical professionals to develop and achieve recovery and life goals. Create and implement goaloriented treatment plans tailored to each patient's unique needs and aspirations. Conduct weekly individual sessions with clients and document progress by the end of each shift. Identify and connect patients to appropriate community resources, advocating for services as needed. Facilitate group sessions that align with program goals and patient recovery needs. Provide leadership, mentorship, and advocacy to patients, colleagues, and the wider community. Respond effectively to crises, delivering intervention services as necessary. Support diverse pathways to recovery, with an emphasis on MedicationAssisted Treatment (MAT) and TraumaInformed practices. Assist with key tasks, including medications management, transportation, admissions, discharges, drug testing, and programmatic activities. Participate in household management, including room inspections, provision of supplies, and ensuring compliance with program policies. Assist residents in completing applications to services and complete all caserelated documentation. Perform other tasks as assigned by management. Minimum Qualifications (Knowledge, Skills, and Abilities) o Prior experience in case management, addiction treatment, or related fields is preferred. A personal commitment to and interest in working with Latino men in recovery. o Bilingual/Bicultural preferred, with strong written and verbal Spanish or Portuguese language skills. Demonstrated sensitivity and adaptability to diverse cultural backgrounds and personalities. o Strong organizational, communication, and problem-solving abilities. Proficiency in documenting patient interactions and creating actionable treatment plans. o Flexible, solutions-oriented, and patient-centered approach to recovery. Ability to work collaboratively in a team environment while maintaining professionalism. o Understanding of trauma-informed care and the principles of recovery. Acceptance of diverse recovery pathways, including MAT, and commitment to non-judgmental, trauma informed care.
