Clinical Case Manager
Marian House
Baltimore, Maryland Commute Time Marian House is seeking a Clinical Case Manager to support women and families in our Transitional Family Program. This role combines counseling, case management, and advocacy to help families achieve stability and long-term independence.
What You'll Do:
- Provide weekly counseling and case management to families
- Develop and monitor Individual Recovery Plans (IRPs)
- Support residents with housing, healthcare, and recovery resources
- Coordinate care with internal staff and community partners
- Facilitate intake, goal-setting, and progress reviews
- Maintain accurate, confidential documentation
- Participate in team meetings and on-call rotation
What We're Looking For:
- Bachelor's or Master degree in Social Work, Counseling, Psychology, or related field required
LMSW, LGPC
(or equivalent MD licensure), CPRS Preferred- 3-5 years of counseling/case management experience
- Experience with trauma-informed care, substance use, and co-occurring disorder
