We serve participants aged 16 and older, including older adults and individuals with disabilities by coordinating medical, behavioral health, social, and community-based supports that foster independence, safety, stability, and overall well-being. The Case Manager collaborates with participants, families or representatives, Medical Day Care Centers, community providers, and internal clinical teams to ensure services are timely, coordinated, and compliant with all state and program requirements. Responsibilities Conduct comprehensive in-person assessments, including psychosocial evaluations, SDOH screenings, risk assessments, and person-centered interviews. Complete initial participant visits within required program timelines. Develop individualized, person-centered Plans of Service (POS) based on participant strengths, needs, goals, preferences, and identified risks. Perform reassessments and redeterminations within required timeframes. Ensure all documentation meets program, regulatory, and organizational standards. Educate participants on rights, responsibilities, informed choice, and available support. Coordinate and link participants to appropriate medical, behavioral health, transportation, housing, caregiver, and community-based services. Maintain ongoing contact through monitoring visits and follow-ups. Document all case management activities accurately in required electronic systems. Identify changes in participant condition, emerging risks, or service gaps and escalate concerns appropriately. Maintain conflict-free case management practices by presenting service options objectively. Report and document any incidents in accordance with policy and timelines. Participate in interdisciplinary meetings and collaborate with community partners. Ensure compliance with state regulations, organizational policies, HIPAA, and documentation standards. Attend required training, supervision sessions, and team meetings. Requirements Passion for supporting individuals aged 16 and older, including older adults and individuals with disabilities. Strong interpersonal, communication, and relationship-building skills. Excellent organizational, documentation, and time-management abilities. Experience in case management, medical day care, behavioral health, or human services preferred. Knowledge of community resources, social determinants of health, and service coordination preferred. Ability to conduct in-home, office-based, and field visits as required. Capacity to work independently while collaborating within a multidisciplinary team. Bachelor's or Master's degree in Social Work, Human Services, Psychology, Public, Health, Gerontology, or a related field.
