Full-time Required License LPC or LMSW. Must be fully licensed Job Overview We are seeking a dedicated and compassionate Clinical Case Manager to join our outpatient clinic team. The ideal candidate will possess a strong background in behavioral health, mental health counseling, and patient care within outpatient settings. This role offers an opportunity to coordinate comprehensive case management services, facilitate patient assessments, and support individuals in achieving optimal health outcomes. The Clinical Case Manager will work closely with multidisciplinary teams to ensure seamless patient transitions across care levels, emphasizing patient-centered approaches and adherence to healthcare regulations. Duties 1.Conduct comprehensive needs assessments to identify barriers to treatment, recovery supports, and social service needs. Assist in identifying strengths, needs, abilities, and preferences of the client. Complete service notes, care coordination documentation, and progress updates in accordance with agency policy, payer requirements, and accreditation standards. 2. Collaborate with clinical staff to support the development and implementation of individualized treatment and recovery plans. Assist with safety planning and coordinate crisis services when necessary. 3. Monitor client progress toward treatment and recovery goals and report updates to the treatment team. 4. Coordinate services between mental health providers, substance use treatment providers, medical providers, and community resources. 5. Facilitate referrals to community services including housing, employment, education, transportation, healthcare, and social services. Assist individuals in navigating systems of care and accessing available resources. 6. Maintain communication with family members and support systems when appropriate and authorized. Communicate regularly with therapists, medical providers, peer support staff, and program leadership. 7. Provide psychoeducation and recovery support related to mental health, substance use, and wellness. Utilize trauma-informed and recovery-oriented approaches when working with persons served

$34.76 - $40.00 per hour
