Job Description

Job Purpose The Physician Case Manager Team Lead (PCMTL) oversees daily workflow and performance of the PCM team, ensuring quality care for individuals receiving medication-only services. This role provides case management support, monitors compliance with documentation requirements, collaborates with the treatment team and Director, and conducts chart audits to support program quality and improvement. Qualifications Associate's degree in humanities with behavioral health experience/training, or Bachelor's degree in a related field with relevant experience. Core Job Duties Provide daily oversight and coordination of the PCM team's activities. Serve as a mentor and role model for effective engagement, resource linkage, and referral practices. Communicate regularly with the Director regarding key activities and updates. Conduct chart audits to ensure compliance with documentation standards. Collaborate with clients to develop individualized treatment plans with measurable goals and objectives. Monitor and evaluate progress toward established treatment outcomes. Connect clients with community resources as recommended by the Psychiatrist. Develop safety plans incorporating natural supports and review their ongoing effectiveness. Provide lethal means counseling as appropriate and support restriction efforts when high risk is identified. Participate in weekly staffing meetings to review caseloads and levels of care, presenting cases clearly and concisely. Support clients in developing or enhancing interpersonal, social, family, community, and independent living skills. Follow up on missed appointments via phone or written communication and document re-engagement efforts within 72 hours. Collaborate in discharge planning with the Psychiatrist and clients, including outreach to those not actively engaged in services. Adherence to all applicable evidence-based practice models. Orally communicate information effectively and accurately. Assure program operates within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care. Abide by program and agency policies and procedures. Other job duties and special projects as assigned.
