Case Management Experience; Medical experience; experience working with youth; experience working with at risk populations
Minimum :
Bachelor's Degree; at least 21 years of age; able to obtain a Class E driver's license
Position Summary :
Provide medical case management services for residents in all Transitional Living Programs.
Special Working Conditions :
Exposure to residents and at times, treatment conditions. Requires ability to manage crisis situations, emergency situations, and provide trauma informed interventions.

Director of Transitional Living Description of Responsibilities :
The Medical Case Manager will maintain routine health screenings for residents. The Medical Case Manager will communicate with youth and TLS staff to determine when youth need medical or mental health appointments and schedule them accordingly, as well as document resident medical histories. The Medical Case Manager is responsible for documentation of residents' medical appointments as well as communicating pertinent information with the case managers and other team members. The Medical Case Manager communicates with doctors, the pharmacy, and TLS staff to ensure doctors' orders are followed. The Medical Case Manager will develop and maintain positive relationships with area health providers. The Medical Case Manager will advise residents on matters of health, hygiene and safety. The Medical Case Manager transports to and attends residents' medical appointments. The Medical Case Manager will provide additional training to the staff and residents on matters important to health and safety. The Medical Case Manager maintain resident MAR's and Monthly Medical Logs. The Medical Case Manager will treat all residents and staff with respect and fairness, contributing to a safe environment and providing trauma informed care. The Medical Case Manager will conduct oneself in a professional manner in all interactions with staff, residents, family support teams, and others involved in the resident's life. The Medical Case Manager will follow agency policy and procedure. The Medical Case Manager will perform additional duties as directed by the Director of Transitional Living.
