We are Medicaid-contracted and deeply committed to compassionate, evidence-based care within our community. Position summary We are looking for a dedicated Case Manager to join our clinical team. In this role, you will provide individualized case management services to clients enrolled in our IOP program — coordinating care, removing barriers to treatment, and supporting long-term recovery. You will work closely with our Clinical Director, Coordinator of Care, and BHT staff to ensure each client receives wraparound support throughout their time in our program. Key responsibilities Conduct comprehensive needs assessments and develop individualized service plans for clients Coordinate referrals to community resources including housing, transportation, employment, and social services Maintain accurate, timely documentation in the EMR (Kipu) in compliance with Medicaid and DHSR standards Collaborate with the clinical team on treatment planning, discharge planning, and continuity of care Serve as a primary point of contact for clients navigating external systems and benefits (Medicaid, DSS, housing, etc.) Participate in team meetings, case consultations, and required trainings Maintain professional boundaries and adhere to all HIPAA, licensure, and organizational compliance requirements Qualifications Experience in case management, social services, behavioral health, or a related field preferred CADC, Peer Support Specialist certification, or other relevant credential is a plus — but not required Lived experience with recovery is valued and respected Familiarity with NC Medicaid, Vaya Health, or community behavioral health systems is a plus Strong organizational skills and attention to documentation detail Ability to work collaboratively within a multidisciplinary team Reliable transportation and valid NC driver's license Compensation & benefits
Salary:
$35,360 - $39,520 annually ($17-$19/hr equivalent) Full-time, salaried position Paid time off Supportive, mission-driven team environment Professional development and continuing education opportunities
