Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness, co-occurring developmental disabilities and substance use disorders. The Adult Community Support Clinician provides diagnostic assessment, treatment planning, therapy, and crisis management. This is a full-time position. Major Functional Areas and Description of Duties Provide individual diagnostic assessment to determine client's strengths, needs, resources, and objectives. Formulate treatment plans specifying presenting problems, objectives, interventions, and desired outcomes. Complete assessments for eligibility determination for State funded services. Provide individual and group therapy, crisis management, and community-based support in a variety of settings including the home, public, office or remote via telehealth. Provide crisis management and intervention when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community. Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care. Provide timely documentation on each service delivered. Provide set hours of billable service each week to meet key performance indicator expectations and eligible for incentive pay upon exceeding productivity expectation. Engage in appropriate consultation meetings and committees, both internal and external, as determined by supervisor. Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients. Execute all required services and documentation in accordance with agency policies and federal/state standards. Maintain professional memberships, licenses, accreditation, certifications, etc. Represent CLM in a professional and appropriate manner in all settings. Perform all other duties as assigned by supervisor. Physical Demands and Working Conditions The work will be performed in the office, in the client's home, out in the community and/or remote via telehealth. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable internet access, transportation, a clean driving record, and proof of auto insurance coverage is required. Requirements:

Qualifications and Skills Master's degree required LCMHC or LICSW preferred Experience working with co-occurring disorders preferred Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations Proficient computer skills, specifically with all Microsoft office applications Ability to document effectively and efficiently in course of clinical work Ability to navigate an electronic health record Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services Effective communication skills, organizational skills, and ability to multi-task Therapist, Adult Community Support Department
