Behavioral Health Clinical Case Manager - Field in Ocean and Atlantic Counties, NJ (Telework + Field-Based Travel) Join our LTSS team as a Behavioral Health Clinical Case Manager, where you'll play a vital role in improving the lives of our members by connecting them with the care, resources, and support they need to thrive. This is a unique opportunity to blend clinical expertise with care coordination in a flexible, telework environment—while still getting out into the community to make a real impact. As a Clinical Case Manager, you will: Conduct comprehensive, face-to-face assessments for members enrolled in MLTSS and D-SNP programs Develop and implement personalized care plans that address physical, behavioral, and psychosocial needs Collaborate closely with members, families, providers, and interdisciplinary teams to coordinate care Evaluate and facilitate services such as: Adult and pediatric medical daycare Personal care assistance Nursing facility support Community-based and long-term services Advocate for members to ensure access to appropriate and high-quality care Authorize services and ensure efficient, cost-effective resource utilization Document care activities accurately in the electronic health record Participate in interdisciplinary meetings and help drive positive outcomes Support and mentor new team members as you grow in your role What Makes This Role Special Flexible telework with meaningful in-person community engagement Opportunity to build lasting relationships with members and providers Be a trusted advocate for vulnerable populations Continuous development, including NJ Choice Certification support A role where your clinical judgment and critical thinking truly matter
Required Qualifications:
Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) Minimum 2+ years of behavioral health experience Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy Ability to successfully complete NJ Choice Certification (required for continued employment)Preferred Qualifications:
Experience in case management or discharge planning Managed care experience Strong crisis intervention skillsEducation:
Master's Degree (required) Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $66,575.00 - $142,576.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
