Full Job Description Position Overview:
The Wraparound Therapist at Family Promise of Greater Cleveland (FPGC) provides comprehensive, trauma-informed services combining therapy and case management. This role is crucial for delivering integrated behavioral health and housing support services to individuals and families from shelter intake through community integration. The Wraparound Therapist will provide wraparound services to support clients and families in community stability.
Principal Duties and Responsibilities:
Therapeutic Services:
Conduct diagnostic assessments and provide ongoing therapy to individuals and families in shelter and community settings. Facilitate clinical and enrichment groups using a strengths-based, trauma-informed and responsive approach.
Case Management:
Develop and periodically update individualized service plans (ISPs) focusing on mental health needs, housing stability, and economic self-sufficiency. Coordinate with other services and providers, monitor progress, and adjust plans as necessary to ensure effective outcomes.
Crisis Support:
Assist in crisis management and stabilization; develop and coordinate implementation of crisis management plans.
Community and Resource Coordination:
Assess needs relating to psychiatric, physical health, social services, and community resources; assist clients in accessing these services. Build and maintain relationships with external agencies and service providers to support housing and service needs.
Documentation and Compliance:
Maintain accurate records of client interactions, progress, and service provision in compliance with agency and regulatory standards. Participate in quality assurance, program evaluation, and seek CARF accreditation.
Professional Development:
Attend and participate in required training, staff meetings, workshops, and supervision. Engage in ongoing professional development relevant to the role.
Qualifications:
Bachelor's degree required; Master's degree in social work, psychology, or related field preferred. Licensure as
LSW, LISW, LPC, LPCC, LPCC-S, MFT, IMFT, IMFT-S
required. At least one year of experience in behavioral health therapy and case management. Strong clinical skills and commitment to trauma-informed care.

Demonstrated clinical proficiency in assessment and on-going services. Mission driven with a heart to serve Knowledge of relevant ethical codes, laws and professional standards The ability to work independently and collaboratively Detail oriented, organized, focused and goal oriented High level of initiative and energy, as well as problem solving skills Proficient in Microsoft Office, including Word, Excel and PowerPoint and Apple products Excellent verbal, written and digital communication skills Automobile and valid driver's license.
