HR JOB SUMMARY
  • We are seeking a dedicated professional to join our Homeless Services Division as a Field Case Manager.
  • This full-time, contract position involves providing community-based support and services to individuals, primarily in the field.
  • The role requires interaction with a multidisciplinary team to ensure participants receive personalized services to maintain housing and achieve recovery goals.
  • Proficiency in general office software is necessary.
Requirements:
  • Conduct home-based visitations and community engagement.
  • Coordinate with a multidisciplinary team for service delivery.
  • Ensure compliance with service contracts and regulations.
  • Develop and monitor personal goal plans for participants.
  • Conduct outreach to engage authorized participants.
  • Provide clinical services or assist participants in accessing services.
  • Maintain effective professional relationships within the community.
Qualifications:
  • Bachelor's degree in a relevant field such as sociology, social work, psychology, etc.
  • Strong communication skills, both verbal and written.
  • Ability to establish supportive relationships with individuals with mental illnesses.
  • Capability to work independently and as part of a team.
  • Proficiency in general office software (e.g., word processing, spreadsheets).
  • Experience with electronic health records preferred.
  • Valid driver's license with an acceptable driving record.
