HR JOB SUMMARY
- We are seeking a dedicated professional to join our Homeless Services Division as a Field Case Manager.
- This full-time, contract position involves providing community-based support and services to individuals, primarily in the field.
- The role requires interaction with a multidisciplinary team to ensure participants receive personalized services to maintain housing and achieve recovery goals.
- Proficiency in general office software is necessary.
Requirements:
- Conduct home-based visitations and community engagement.
- Coordinate with a multidisciplinary team for service delivery.
- Ensure compliance with service contracts and regulations.
- Develop and monitor personal goal plans for participants.
- Conduct outreach to engage authorized participants.
- Provide clinical services or assist participants in accessing services.
- Maintain effective professional relationships within the community.
Qualifications:
- Bachelor's degree in a relevant field such as sociology, social work, psychology, etc.
- Strong communication skills, both verbal and written.
- Ability to establish supportive relationships with individuals with mental illnesses.
- Capability to work independently and as part of a team.
- Proficiency in general office software (e.g., word processing, spreadsheets).
- Experience with electronic health records preferred.
- Valid driver's license with an acceptable driving record.
