This vital role involves coordinating comprehensive case management services to ensure seamless patient transitions across inpatient, outpatient, and post-acute care settings. You will leverage your extensive clinical knowledge and compassionate approach to facilitate optimal health outcomes, advocate for patients, and streamline healthcare processes. Duties Conduct thorough patient assessments, including medical history review, physiological evaluation, and psychosocial factors to develop personalized care plans. Coordinate discharge planning by collaborating with multidisciplinary teams, ensuring timely and appropriate patient discharges from hospital or clinic settings. Manage utilization review and utilization management processes to optimize resource use while maintaining quality standards. Facilitate diagnostic evaluations and coordinate post-acute care services such as home health, rehabilitation, or outpatient clinics. Maintain accurate and confidential medical records in compliance with HIPAA regulations; document all case management activities meticulously. Provide patient education and support regarding treatment options, medication adherence, and community resources to empower informed decision-making. Liaise with insurance providers and managed care organizations to authorize services and streamline authorization processes. Skills Proven experience in hospital settings with expertise in Level I Trauma Centers, Level II trauma centers, or similar acute care environments. Strong background in hospital medicine, emergency medicine, ICU, PICU (Pediatric Intensive Care Unit), or behavioral health settings. Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and inpatient/outpatient care workflows. Familiarity with HIPAA compliance, medical records management, intake procedures, discharge planning, and utilization management protocols. Excellent patient service skills with the ability to perform comprehensive patient assessments and coordinate complex care plans effectively. Experience in addiction counseling or mental health counseling is a plus; familiarity with post-acute care pathways highly desirable. Ability to navigate managed care systems efficiently while advocating for patient needs across diverse healthcare environments.
