Key Responsibilities:
  • Help clients navigate the healthcare system through education, coaching, advocacy, and ongoing support
  • Coordinate behavioral health and primary care services while helping clients overcome barriers to wellness and recovery
  • Develop, implement, monitor, and update individualized treatment plans in collaboration with clients and families
  • Connect clients with community resources, supportive services, and natural support systems to promote long-term stability
  • Utilize motivational interviewing, active listening, behavioral management techniques, and crisis intervention skills to support client success
  • Provide supportive case management services focused on developing daily living, coping, and life management skills
  • Meet with clients in the office, community, telehealth platform, and by phone to assess needs and provide ongoing support
  • Complete timely and accurate documentation within the Electronic Medical Record for all services provided
  • Participate in team meetings, staffing discussions, assessments, and clinical supervision activities
  • Maintain departmental productivity expectations and compliance with required trainings and organizational standards
  • Demonstrate professionalism and ethical practice when working with clients, families, and community partners Education, Experience, and/or
Credential Qualifications:
  • Bachelor's degree in psychology, social work, counseling, or a related field required
  • Experience working with Electronic Health Records preferred
  • Experience in behavioral health, case management, community mental health, care coordination, crisis intervention, or social services preferred
Additional Qualifications:
  • Valid driver's license in good standing required
  • Demonstrate strong communication, organization, and time management skills
  • Ability to work independently while also collaborating effectively within a multidisciplinary treatment team
  • Ability to build rapport with clients and families from diverse backgrounds
  • Demonstrate compassion, professionalism, flexibility, and sound judgment in a fast-paced environment
  • Basic computer proficiency and documentation skills required Physical Requirements
  • Sit for extended periods throughout the workday
  • Stand and move throughout office and community settings as needed
  • Occasionally squat, kneel, bend, lift, and drive for work-related responsibilities
  • Perform physical tasks associated with community-based case management and client support responsibilities
