Status Neutral Retention Case Manager FLSA Status:

SUMMARY:
Seeking a compassionate, responsible and mature individual to fill Status Neutral Retention Case Manager and will work within the prevention department to ensure continuity and expansion of the PrEP education, engagement, patient navigation and adherence-counseling program at Valley AIDS Council (VAC) and its Westbrook Clinics. The Retention Case Manager will be responsible for providing retention services to PrEP clients. The Retention Case Manager will provide clients with adherence support, address barriers to access prevention services, phone call reminders to appointments, and follow-ups to medical appointments schedule of labs. Retention Case Managers will ensure that services delivered within and related to this intervention are services that are culturally competent and reflective of the most up to date HIV/STI-related education including sexual health information and the provision of referrals. Works with other members of Health Promotion and Community Engagement to achieve organizational goals.
DUTIES AND RESPONSIBILITIES
1. Assist in the continued implementation of PrEP services and appointment scheduling at Westbrook Clinics, while also ensuring that individuals at higher vulnerability for HIV acquisition receive PrEP education, referrals, tested for HIV/STIs and obtain PrEP follow up medical appointment. 2. The Retention Case Manager will have a caseload of clients taking PrEP and will conduct client-centered PrEP education and adherence counseling services with a focus on patient acquisition, engagement and retention.
  • Participation in outreach activities including with the Education and Prevention Department.
  • Provide patients and clients with basic introductory education regarding PrEP and the process to engage in PrEP Medical Care.
  • Recruit individuals at higher vulnerability for HIV acquisition into PrEP education.
  • Host and participate in town halls or community forums to provide education on prevention and revolving around PrEP.
  • Follow-up services, offering adherence counseling and assistance in scheduling appointments for patients receiving PrEP.
  • Acquire, fulfill, and maintain required documentation for patient assistance programs.
  • Assess and determine if patients are insured and educate them in the value of the 340B Program
  • Ensure proper collection, storage, and transmission of data used in reporting, research, and program evaluation.
  • Follow-up on non-compliant PrEP clients to bring back into care to access PrEP services.
3. Provide individual and relationship-based HIV and STI counseling and testing, including results delivery for point-of-care ("rapid") and laboratory-based tests, and appropriate referral based services. 4. Document visits and interactions appropriately within VAC's Electronic Health Records System (EHRS). 5. Facilitate and/or provide "finger-stick" rapid HIV testing and venous blood draws (i.e. phlebotomy) for other tests. 6. Must follow OSHA and blood-borne pathogen guidelines at all times. 7. Provide education about Post-Exposure Prophylaxis (PEP), and other effective biomedical and prevention interventions and refer eligible, interested clients to obtain appropriate medical follow up services at Westbrook Clinic to further discuss PEP and other preventive care. 8. Work with the Testing and Education Coordinator to revise, assist, and communicate changes and recommendations for PrEP services. 9. Assist the Testing and Education Coordinator in providing the training of frontline staff, education staff, volunteers, interns and other members of VAC/Westbrook Clinic as appropriate. 10. Design, implement, and distribute advertising and educational campaigns on PrEP in the Rio Grande Valley. 11. Coordinate collaborative relationships related to PrEP implementation and service delivery, focusing on agencies located in and around South Texas. 12. Accurately complete all statistical data forms for PrEP, primarily within VAC's Electronic Health Records System, ensuring complete and accurate information reimbursement for services rendered. 13. Review and maintain up-to-date and accurate knowledge of emerging scientific research regarding biomedical HIV prevention strategies, including PrEP, PEP and related interventions. 14. Provide HIV and STI testing as needed. 15. Must attend agency wide events including but not limited to World AIDS Day, Back to School, VIDA patient conference, etc. 16. Position requires a flexible work schedule that includes evenings/weekends and extended hours. 17. Outreach activities occur outside normal working hours (8:00 AM - 5:00 PM, Monday through Friday). 18. Attend departmental meetings, organizational meetings and mandatory trainings. 19.Maintain highest level of client confidentiality and abide to federal HIPAA regulations and VAC policies. 20. Must follow all agency rules and regulations including but not limited to Employee Handbook and safety protocols and programmatic. 21. Must cross-collaborate within the Health Promotion and Community Engagement departments to better serve our clientele/patients and achieve organizational and departmental goals. 22. Perform other duties as required and assigned.
QUALIFICATIONS
High School Diploma or GED. Associates degree in Social Work, psychology, social services, or related field preferred. Course work in Social work, psychology or related field plus 1 to 2 years of experience in non-profit ambulatory service organization working with vulnerable populations. 1 year experience in Protocol Based Counseling/Intervention or Foundations of HIV Testing and navigation preferred. Education may be substituted with experience. Medical Assistant and Phlebotomy skills and experience a great plus. Certification or Registration preferred. CHW experience and certification highly desired. Prefer one year of case management experience with people living with HIV, and/or persons with a history of mental illness, homelessness, or chemical dependence; or any equivalent combination of education and experience. Experience in non-profit, healthcare setting preferred. Must be able to work under conditions of constant interruption and be able to stay on task and attention to detail. Compassionate and empathetic with ability to relate to target population from all walks of life. Ability to communicate effectively and with tact highly sensitive information. Ability to participate effectively as a team member and contribute to a positive work environment. Knowledge of
HIV/AIDS
; knowledge of community resources and referrals; knowledge of OSHA regulations. Working knowledge of Microsoft Office, specifically Outlook, Word & Excel and Teams. Basic math skills to perform arithmetic calculations. Maintain a flexible schedule Monday - Friday, may occasionally require non-traditional hours, such as extended hours, evenings and weekends. Must have oral and written communication skills in English; organizational and time management skills. Ability to perform multiple tasks; good problem-solving abilities; demonstrate flexibility and people skills. Ability to work with people of diverse groups/backgrounds. Bilingual (English-Spanish) to effectively communicate with population served. Knowledge and ability to safely operate a vehicle. Active Texas Class "C" Driver's License and automobile liability insurance. Must maintain all certifications, licenses and/or registrations consistent with Westbrook Clinic policy and Texas State Law. Requires frequent use of desktop computer, including word processing and spreadsheet programs; general office equipment and operating an automobile.

Frequent sitting and communication requirements. Walking and use of hands to finger, handle, or feel objects, tools, or controls; and reach with hands and arms. The employee must occasionally lift and/or move up to 35 pounds with or without assistance. Specific vision abilities required by this job include close vision and the ability to adjust focus. Drive from site to site and home visits. Work is predominantly performed in an office setting, confined space where the noise level is usually quiet.
