Non-Exempt Position Summary:
The Case Manager is responsible for coordinating services and support for adolescent clients participating in residential substance abuse treatment. This position ensures individualized treatment planning, continuity of care, and effective discharge planning while collaborating with a multidisciplinary team to meet the physical, emotional, educational, and social needs of each client.
Key Responsibilities:
Conduct initial intake assessments and psychosocial evaluations. Develop and maintain individualized treatment plans in collaboration with the clinical team and client guardians. Provide case management services, including referrals for medical, mental health, educational, legal, and social support services. Coordinate and lead treatment team meetings to review client progress and adjust plans as necessary. Maintain regular communication with parents, guardians, and external agencies (schools, probation officers, CPS workers, etc.). Ensure all documentation is accurate, complete, and compliant with state regulations (Texas Health and Human Services - HHSC) and facility policies. Advocate for client needs both within the program and with external service providers. Participate in interdisciplinary staff meetings, clinical supervision, and required trainings. Develop and implement discharge plans to ensure continuity of care and successful reintegration into the community or next level of care. Maintain confidentiality in compliance with
HIPAA, 42 CFR
Part 2, and facility policies. Support a trauma-informed, culturally competent, and adolescent-centered approach to care. Perform other duties as assigned by supervisor.
Qualifications:
Bachelor's Degree in Social Work, Psychology, Counseling, or related human services field (required). Licensed Chemical Dependency Counselor (LCDC), Licensed Professional Counselor Associate (LPC-A), or related license preferred; LCDC-I considered. Minimum of 1 year of experience working with adolescents and/or substance abuse populations preferred. Knowledge of adolescent development, substance use disorders, trauma-informed care, and community resources. Strong organizational skills and ability to manage a caseload effectively. Ability to work independently and as part of a multidisciplinary team. Proficient in Microsoft Office and Electronic Health Records (EHR) systems. Must pass criminal background check and drug screening per state requirements. CPR/First Aid Certification (or willing to obtain within 30 days of hire).
Work Environment:
Residential treatment setting for adolescents aged 13-17. Frequent interaction with clients exhibiting behavioral and emotional challenges. Fast-paced, team-oriented environment requiring flexibility and resilience. Evening and weekend availability may be required for family meetings or emergencies.
