In this vital role, you will assess patient needs, develop comprehensive care plans, and collaborate with multidisciplinary teams to optimize health outcomes. Your critical thinking and compassionate approach will empower patients on their journey to recovery and well-being. This paid position offers an exciting opportunity to make a meaningful impact in diverse clinical environments, including hospitals, emergency departments, and specialized care units. Duties Conduct thorough patient assessments to determine medical and psychosocial needs using EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems. Develop individualized discharge planning strategies that facilitate smooth transitions from hospital to home or other care facilities. Coordinate with healthcare providers, social workers, and family members to ensure comprehensive patient support. Perform utilization review and management to verify appropriate hospital stays, procedures, and services in compliance with HIPAA regulations. Manage case documentation accurately, including CPT coding, ICD-9/10 coding, and medical terminology entries for billing and reporting purposes. Facilitate intake processes for new patients requiring behavioral health, hospice care, or pediatric services within Level I or Level II trauma centers. Support hospital medicine teams by providing critical case management in acute care settings such as
ICU, PICU
(Pediatric Intensive Care Unit), OR (Operating Room), and emergency medicine units. Requirements Proven experience in critical care, hospital settings, or emergency medicine with a strong understanding of acute care protocols. Familiarity with EMR/EHR systems and healthcare compliance standards including HIPAA regulations. Experience working within managed care environments and performing discharge planning or utilization management tasks. Excellent communication skills to collaborate effectively with multidisciplinary teams across various hospital departments such as ICU, PICU, trauma centers, and hospice programs. Ability to handle complex cases involving pediatrics, trauma patients, or patients requiring intensive support in Level I or Level II trauma centers.
